There have been many changes since EfiLoadDxe.cmm was last updated in
2011. The EFI_SYSTEM_TABLE can no longer be found by scanning memory on
4KB boundaries, so require users pass in its address instead. Update
various offsets so that the debug information can be found and loaded
with a recent version of TRACE32.

This file contained what looked like windows 1250 encoded single
quotation marks. Convert them to regular 's. Rework the file to
markdown format and and rename it Readme.md while we're at it.

Problems have been encountered because some of the source files have
execute permission set. This can cause git to report them as changed
when they are checked out onto a file system with inherited permissions.
This has been seen using Cygwin, MinGW and PowerShell Git.
This patch makes no change to source file content, and only aims to
correct the file modes/permissions.

These scripts are used for UEFI Source debugging with Lauterbach T32.
They can be used at different stages of the UEFI boot time.
