17e67d26d9 MdeModulePkg DxeMain: Add late initialization for Debug Agent.
Add a late initialize in DxeMain for the debug agent. This is required
for the debug agent to be able to setup events to handle image loads,
exit boot services, and other important callbacks.

Define a reinitialize debug agent.

e8668d2dee MdeModulePkg/DxeCore: Call BeforeExitBootServices event group only once
According to UEFI spec 2.10 errata A section 7.4.6

  "All events from the EFI_EVENT_GROUP_BEFORE_EXIT_BOOT_SERVICES and
  EFI_EVENT_GROUP_EXIT_BOOT_SERVICES event notification groups as well
  as events of type EVT_SIGNAL_EXIT_BOOT_SERVICES must be signaled
  before ExitBootServices() returns EFI_SUCCESS. The events are only
  signaled once even if ExitBootServices() is called multiple times."

So keep track of whether ExitBootServices() has been called, and signal
the event group EFI_EVENT_GROUP_BEFORE_EXIT_BOOT_SERVICES only the first
time around.

EFI_EVENT_GROUP_EXIT_BOOT_SERVICES will only be signalled if
ExitBootServices() is going to run to [successful] completion, after
which calling it a second time is not possible anyway. So for this case,
no special handling is needed.

0596e5fa05 MdeModulePkg: CoreValidateHandle Optimization

Before entering BIOS setup, CoreValidateHandle function executed
over 600,000 times during BDS phase on latest 8S server platform.
In CoreValidateHandle function, current implementation will go
through the doubly-linked list handle database in each call, and
this will have big impact on boot performance.
The optimization is using Red-black tree to store the EFI handle
address when insert each EFI handle into the handle database, and
remove the handle from Red-black tree if the handle is removed
from the handle database. CoreValidateHandle function changed to
go through the Red-black tree.
After verification on latest 8S server platform, BDS boot time can
save 20s+ after this change.

c5e702e45a MdeModulePkg/Core/Dxe: Initialize GCD before RT memory allocations
Update the DxeMain initialization order to initialize GCD
services before any runtime allocations are performed.  This
is required to prevent runtime data fragmentation when the
UEFI System Table and UEFI Runtime Service Table are allocated
before both the memory and GCD services are initialized.

7b82da70ed MdeModulePkg: Update code to be more C11 compliant by using __func__
__FUNCTION__ is a pre-standard extension that gcc and Visual C++ among
others support, while __func__ was standardized in C99.

Since it's more standard, replace __FUNCTION__ with __func__ throughout
MdeModulePkg.

6676162f64 DxeMain: Fix the bug that StackGuard is not enabled
Commit e7abb94d1 removed InitializeCpuExceptionHandlersEx
and updated DxeMain to call InitializeCpuExceptionHandlers
for exception setup. But the old behavior that calls *Ex() sets
up the stack guard as well. To match the old behavior,
the patch calls InitializeSeparateExceptionStacks.

e7abb94d1f CpuException: Add InitializeSeparateExceptionStacks
Today InitializeCpuExceptionHandlersEx is called from three modules:
1. DxeCore (links to DxeCpuExceptionHandlerLib)
    DxeCore expects it initializes the IDT entries as well as
    assigning separate stacks for #DF and #PF.
2. CpuMpPei (links to PeiCpuExceptionHandlerLib)
   and CpuDxe (links to DxeCpuExceptionHandlerLib)
    It's called for each thread for only assigning separate stacks for
    #DF and #PF. The IDT entries initialization is skipped because
    caller sets InitData->X64.InitDefaultHandlers to FALSE.

Additionally, SecPeiCpuExceptionHandlerLib, SmmCpuExceptionHandlerLib
also implement such API and the behavior of the API is simply to initialize
IDT entries only.

Because it mixes the IDT entries initialization and separate stacks
assignment for certain exception handlers together, in order to know
whether the function call only initializes IDT entries, or assigns stacks,
we need to check:
1. value of InitData->X64.InitDefaultHandlers
2. library instance

This patch cleans up the code to separate the stack assignment to a new API:
InitializeSeparateExceptionStacks().

Only when caller calls the new API, the separate stacks are assigned.
With this change, the SecPei and Smm instance can return unsupported which
gives caller a very clear status.

The old API InitializeCpuExceptionHandlersEx() is removed in this patch.
Because no platform module is consuming the old API, the impact is none.

1366cd58cd MdeModulePkg/Core/Dxe: log memory base and length, after lib ctors again
CoreInitializeMemoryServices() logs "BaseAddress" and "Length" with
DEBUG() before DxeMain() calls ProcessLibraryConstructorList()
explicitly. (Library construction is not an automatic part of the DXE
Core entry point.)

So those DEBUG()s in CoreInitializeMemoryServices() are issued against
an un-constructed DebugLib, and also against a -- possibly underlying --
un-constructed SerialPortLib.

Some DebugLib instances can deal with this (see for example commit
91a5b1365075, "OvmfPkg/PlatformDebugLibIoPort: fix port detection for
use in the DXE Core", 2018-08-06), while some others can't (see for
example the DebugLib instance
"MdePkg/Library/BaseDebugLibSerialPort/BaseDebugLibSerialPort.inf"
coupled with the SerialPortLib instance
"ArmVirtPkg/Library/FdtPL011SerialPortLib/FdtPL011SerialPortLib.inf").

Addressing this issue in a SerialPortLib instance that underlies
BaseDebugLibSerialPort seems wrong; either the DebugLib instance should
cope directly with being called un-constructed (see again commit
91a5b1365075), or the DXE Core should log relevant information *at
least* after library instances have been constructed. This patch
implements the latter (only for the "BaseAddress" and "Length" values
calculated by CoreInitializeMemoryServices()).

bce776a5b6 MdeModulePkg/DxeMain: Fix BSP interrupts reenabled in ExitBootServices
Within function CoreExitBootServices(), this commit will move the call
of:

MemoryProtectionExitBootServicesCallback();

before:

SaveAndSetDebugTimerInterrupt (FALSE);
and
gCpu->DisableInterrupt (gCpu);

The reason is that, within MemoryProtectionExitBootServicesCallback(),
APIs like RaiseTpl and RestoreTpl maybe called. An example will be:

DebugLib (using PeiDxeDebugLibReportStatusCode instance)

ReportStatusCodeLib (using DxeReportStatusCodeLib instance)
 |
 v
Raise/RestoreTpl

The call of Raise/RestoreTpl APIs will re-enable BSP interrupts. Hence,
this commit refine the calling sequence to ensure BSP interrupts before
leaving CoreExitBootServices().

a8ab14d355 MdeModulePkg/Core/Dxe: Call new API InitializeCpuExceptionHandlersEx instead
Original API InitializeCpuExceptionHandlers is used in DxeMain to initialize
exception handlers but it does not support setting up stack switch required
by Stack Guard feature. Using the new API instead to make sure Stack Guard
feature is applicable to most part of code.

Since this API is called before memory service initialization, there's no
way to call AllocateXxx API to reserve memory. Global variables are used
for this special case. GDT table is reserved at least 2KB which should be
big enough for all current use cases.

c60370454e MdeModulePkg Core: Propagate PEI-phase FV authentication status to DXE
FV3 HOB was introduced by new (>= 1.5) PI spec, it is intended to
be used to propagate PEI-phase FV authentication status to DXE.
This patch is to update PeiCore to build FV3 HOB with the
authentication status and DxeCore to get the authentication
status from FV3 HOB when producing FVB Protocol.

d0e92aad46 MdeModulePkg/DxeCore: Add UEFI image protection.
If the UEFI image is page aligned, the image code section is set to read
only and the image data section is set to non-executable.

1) This policy is applied for all UEFI image including boot service driver,
runtime driver or application.
2) This policy is applied only if the UEFI image meets the page alignment
requirement.
3) This policy is applied only if the Source UEFI image matches the
PcdImageProtectionPolicy definition.
4) This policy is not applied to the non-PE image region.

The DxeCore calls CpuArchProtocol->SetMemoryAttributes() to protect
the image. If the CpuArch protocol is not installed yet, the DxeCore
enqueues the protection request. Once the CpuArch is installed, the
DxeCore dequeues the protection request and applies policy.

Once the image is unloaded, the protection is removed automatically.

The UEFI runtime image protection is teared down at ExitBootServices(),
the runtime image code relocation need write code segment at
SetVirtualAddressMap(). We cannot assume OS/Loader has taken over
page table at that time.

NOTE: It is per-requisite that code section and data section
should not be not merged. That is same criteria for SMM/runtime driver.

We are not able to detect during BIOS boot, because
we can only get LINK warning below:
"LINK : warning LNK4254: section '.data' (C0000040) merged into
'.text' (60000020) with different attributes"
But final attribute in PE code section is same.

CpuDeadLoop (). When NORETURN is added for PeiCore(), MSVC compiler will report
warning C4702: unreachable code for CpuDeadLoop (). And, the warning is treated
as error and cause build break. DxeMain() has the similar issue.

edk2 uses EntryPoint library to wrap every module entry point function except
for SEC. The module entry point is still called by _ModuleEntryPoint(). So,
there will be negative impact to add NORETURN for the module entry point.

3a7daf9eb0 MdeModulePkg/Core: Decorate phase-transition functions with NORETURN.
This patch adds the NORETURN attribute to functions that transfer to
other phases, along with an UNREACHABLE() call at the end to avoid
false warnings.
DxeIpl has been excluded as its main function returns a status.

dfbc039fac MdeModulePkg/DxeCore: set ImageContext Handle and ImageRead() fields
Set the ImageContext.Handle and ImageContext.ImageRead() fields so that
PeCoffLoaderRelocateImageExtraAction() can invoke PeCoffLoaderGetImageInfo
or PeCoffLoaderGetPeHeader if desired to obtain additional metadata.

We will use this to create a PeCoffLoaderRelocateImageExtraAction()
implementation that applies boot time strict mapping permissions to
PE/COFF modules.

74a8877033 MdeModulePkg DxeCore: Enhance MemoryAttributesTable installation
Current MemoryAttributesTable will be installed on ReadyToBoot event
at TPL_NOTIFY level, it maybe incorrect when PcdHiiOsRuntimeSupport
= TRUE as HiiDatabaseDxe will have runtime memory allocation for HII
OS runtime support on and after ReadyToBoot. The issue was exposed at
http://article.gmane.org/gmane.comp.bios.edk2.devel/10125.

To make sure the correctness of MemoryAttributesTable, this patch is
to enhance MemoryAttributesTable installation to install
MemoryAttributesTable on ReadyToBoot event at TPL_CALLBACK - 1 level
to make sure it is at the last of ReadyToBoot event, and also hook
runtime memory allocation after ReadyToBoot.
