Marcin Wojtas e5cd809087 MdeModulePkg: XhciDxe: Prevent illegal memory access in XhcSetHsee

Newly added XhcSetHsee() routine reads 4 bytes into
a UINT16 variable causing issues on PCIE and NonDiscoverable
Xhci controllers. Fix that.

Star Zeng 73dbd6afab MdeModulePkg XhciDxe: Set HSEE Bit if SERR# Enable Bit is set

When the HSEE in the USBCMD bit is a ‘1’ and the HSE bit in the
USBSTS register is a ‘1’, the xHC shall assert out-of-band error
signaling to the host and assert the SERR# pin.
To prevent masking any potential issues with SERR, this patch is
to set USBCMD Host System Error Enable(HSEE) Bit if PCICMD SERR#
Enable Bit is set.

Hao Wu 16f6922709 MdeModulePkg: Refine casting expression result to bigger size
There are cases that the operands of an expression are all with rank less
than UINT64/INT64 and the result of the expression is explicitly cast to
UINT64/INT64 to fit the target size.

An example will be:
UINT32 a,b;
// a and b can be any unsigned int type with rank less than UINT64, like
// UINT8, UINT16, etc.
UINT64 c;
c = (UINT64) (a + b);

Some static code checkers may warn that the expression result might
overflow within the rank of "int" (integer promotions) and the result is
then cast to a bigger size.

The commit refines codes by the following rules:
1). When the expression is possible to overflow the range of unsigned int/
int:
c = (UINT64)a + b;

2). When the expression will not overflow within the rank of "int", remove
the explicit type casts:
c = a + b;

3). When the expression will be cast to pointer of possible greater size:
UINT32 a,b;
VOID *c;
c = (VOID *)(UINTN)(a + b); --> c = (VOID *)((UINTN)a + b);

4). When one side of a comparison expression contains only operands with
rank less than UINT32:
UINT8 a;
UINT16 b;
UINTN c;
if ((UINTN)(a + b) > c) {...} --> if (((UINT32)a + b) > c) {...}

For rule 4), if we remove the 'UINTN' type cast like:
if (a + b > c) {...}
The VS compiler will complain with warning C4018 (signed/unsigned
mismatch, level 3 warning) due to promoting 'a + b' to type 'int'.

Feng Tian dbe10619bc MdeModulePkg/XhciDxe:1ms delay before access MMIO reg during reset
Some XHCI host controllers require to have extra 1ms delay before
accessing any MMIO register during HC reset.

As this delay is not defined by XHCI spec, we use this workaround
to fix the issue.

Feng Tian 26cd2d6d2d MdeModulePkg/Xhci: make all timeout values be consistent with comments.
In the original code, there exists some mismatches between the real
waiting time and the corresponding timeout comments. For example, the
XHC_GENERIC_TIMEOUT comment says it's 10ms timeout value, but the real
code in fact waits 10s.

So the code is refined to be consistent in code logic and comments.

Note XHC_POLL_DELAY macro also be removed and the polling interval in
XhcWaitOpRegBit() is changed from 1ms to 1us to keep same code style
with other code. It has no real functionality impact.

erictian 6b4483cdbd MdeModulePkg: The patch eliminates two assumptions
1) XHCI host controller hw always provides more than 5 interrupters.
Now using interrupter 0 to accommodate all received events.
2) XHCI host controller hw always provides 32bytes context size.
Now it dynamically detect context size and construct it.

also solved several issues:
1) Divides 64byte width register access to two 32bit registers access because some XHCI chipsets cannot support a single 64bit access.
2) Remove halt host controller statement in UsbBusDriverBindingStop(). It has been done by host controller’s DriverBindingStop(). And XhciDriverBindingStop() need XHCI host controller is in running state because it need execute DISABLE_SLOT cmd to release h/w resource.
