Laszlo Ersek b48ec0e8ab MdeModulePkg/EhciDxe: factor out EhcIsDebugPortInUse() 
The EhcReset(), EhcGetRootHubPortStatus() and EhcDriverBindingStart()
functions need to see whether the host controller (or a specific port on
the host controller) can be accessed, dependent on the controller having
(or the specific port being) an in-use debug port. Because the condition
isn't simple, extract it to a separate function.

Ard Biesheuvel 167c3fb456 MdeModulePkg/EhciDxe: enable 64-bit PCI DMA 
PCI controller drivers must set the EFI_PCI_IO_ATTRIBUTE_DUAL_ADDRESS_CYCLE
attribute if the controller supports 64-bit DMA addressing.

eric_tian a261044cf5 code cleanup: 
1. remove unused macro/remove unnecessary included header file
2. for those values defined by PCI spec, use the naming of definition in IndustryStandard/Pci.h
3. use BITX macro in Base.h rather than define one by self.

EFI_SIGNATURE_16 -> SIGNATURE_16
  EFI_SIGNATURE_32 -> SIGNATURE_32
  EFI_SIGNATURE_64 -> SIGNATURE_64
  EFI_FIELD_OFFSET -> OFFSET_OF
  EFI_MAX_BIT      -> MAX_BIT
  EFI_MAX_ADDRESS  -> MAX_ADDRESS
These macros are not defined in UEFI spec. It makes more sense to use the equivalent macros in Base.h to avoid alias.
