Maggie Chu dc254af6a4 MdeModulePkg/NvmExpressPei: Fix Opal S3 unlock issue

This patch is for fixing unexpected system hang during S3 unlock process.
FatPei driver maintained and updated internal BlockIo devices list
when there is new BlockIo PPI has installed, and it relied on BlockIo PPI
service to get data from devices. Because BlockIo Ppi leverage
NvmExpressPei Ppi to transit Nvm command to device, we should make sure
NvmePassThruPpi installed before BlockIo PPI.

Ashish Singhal 2bbbdeeea2 MdeModulePkg/XhciPei: Fix Aligned Page Allocation
Add support for allocating aligned pages at an alignment higher
than 4K. The new function allocated memory taking into account
the padding required and then frees up unused pages before mapping
it.

As per UFS spec, flag value is stored in the 'last byte' of value
field. Existing code is attempting to read first byte.

Laszlo Ersek 9388c6b1c1 MdeModulePkg: fix UninstallMultipleProtocolInterfaces() calls
Unlike the InstallMultipleProtocolInterfaces() boot service, which takes
an (EFI_HANDLE*) as first parameter, the
UninstallMultipleProtocolInterfaces() boot service takes an EFI_HANDLE as
first parameter.

These are actual bugs. They must have remained hidden until now because
they are on error paths. Fix the UninstallMultipleProtocolInterfaces()
calls.

Albecki, Mateusz 76e1e5631f MdeModulePkg/SdMmcPciHcDxe: Fix SdMmcMmcLegacy bus timing handling

There is a bug in current driver code that makes
it impossible for it to work with devices that
only work in legacy speed mode since the function
EmmcSwitchToHighSpeed will return with EFI_INVALID_PARAMETER
for such bus mode. Since the logic in that function will
work well for SdMmcMmcLegacy this patch just allows
to call this function with SdMmcMmcLegacy bus mode.

SD specification recommends switching card bus timing before
switching bus timing in controller. Emmc driver used to do
this switch other way around. This commit adds controller
timing switch in EmmcSwitchBusTiming function to enforce this
order and removes all controller timing programming from
EmmcSwitchToXXX functions.

Stopping the SD clock before switching host controller
bus timing is only necessary if preset value enable is
set in host controller. Code that performs switch to
HS200 doesn't check this condition right now and we also
don't do the SD clock stop before any other bus timing switch
in host controller. Since the driver is generally not ready to support
host controller with preset value enable set we removed this
code.

Marcin Wojtas 1702e2ce5a MdeModulePkg/UsbBusDxe: Return error when the device is not present
Until now, during the USB device enumeration when its PortState

Albecki, Mateusz adec1f5deb MdeModulePkg/SdMmcHcDxe: Implement revision 3 of SdMmcOverrideProtocol

Implement support for GetOperatingParamters notify phase
in SdMmcHcDxe driver. GetOperatingParameters notify phase
is signaled before we start card detection and initialization.
Code has been updated for both eMMC and SD card controllers to
take into consideration those new parameters. Initialization process
has been divided into 2 steps. In the first step we bring the link
up to the point where we can get card identification data(Extended
CSD in eMMC case and SWITCH command response in SD card case). This
data is later used along with controller capabilities and operating
parameters passed in GetOperatingParameters phase to choose preferred
bus settings in GetTargetBusSettings function. Those settings are later
on to start bus training to high speeds. If user passes incompatible
setting with selected bus timing driver will assume it's standard behavior
with respect to that setting. For instance if HS400 has been selected as a
target bus timing due to card and controller support bus width setting of
4 and 1 bit won't be respected and 8 bit setting will be chosen instead.

Tests on Marvell boards were also performed by Marcin Wojtas

Albecki, Mateusz a37e18f6fc MdeModulePkg/UfsPassThruDxe: Fix unaligned data transfer handling

Since UFS specification requires the data buffer specified
in PRDT to be DWORD aligned in size we had a code in
UfsInitUtpPrdt that aligned the data buffer by rounding down
the buffer size to DWORD boundary. This meant that for SCSI
commands that wanted to perform unaligned data transfer(such as
SENSE command) we specified to small buffer for the data to fit
and transfer was aborted. This change introduces code that allocates
auxiliary DWORD aligned data buffer for unaligned transfer. Device
transfers data to aligned buffer and when data transfer is over driver
copies data from aligned buffer to data buffer passed by user.

Laszlo Ersek e5b4d825af MdeModulePkg/PciBusDxe: catch unimplemented extended config space reads
When assigning a physical PCIe device to a QEMU/KVM guest, PciBusDxe may
find that the extended config space is not (fully) implemented. In
LocatePciExpressCapabilityRegBlock(), "CapabilityEntry" may be read as
0xFFFF_FFFF at a given config space offset, after which the loop gets
stuck spinning on offset 0xFFC (the read at offset 0xFFC returns
0xFFFF_FFFF most likely as well).

Another scenario (not related to virtualization) for triggering the above
is when a Conventional PCI bus -- exposed by a PCIe-to-PCI bridge in the
topology -- intervenes between a PCI Express Root Port and a PCI Express
Endpoint. The Conventional PCI bus limits the accessible config space of
the PCI Express Endpoint, even though the endpoint advertizes the PCI
Express capability. Here's a diagram, courtesy of Alex Williamson:

Catch reads of 0xFFFF_FFFF in LocatePciExpressCapabilityRegBlock(), and

Hao A Wu e2afc8ab59 MdeModulePkg/AhciPei: Fix device cannot be found in non-S3 path

Current implementation of function AhciModeInitialization() has an
incorrect assumption that the value in the CAP (offset 00h) register will
always be greater than the highest bit set for the value in the PI (offset
0Ch) register.

This will lead to an issue that hard disk devices may not be found in the
non-S3 boot path for some AHCI controller capabilities.

More specifically, variable 'PortInitializeBitMap' will have the value
from 'Private->PortBitMap', which will be 0xFFFFFFFF in non-S3 boot path.
When the CAP register is of value 0x1 and PI register with value 0x4
(meaning port 2 is available), the current logic will only enumerate port
0. And the device attached behind port 2 will not be enumerated.

To address this issue, variable 'PortInitializeBitMap' will now take the
bitwise and result between 'Private->PortBitMap' and the value read from
the PI register.

Please note that there will be no function impact for S3 path, since in
this case, the bits being set in 'Private->PortBitMap' will be a subset
of the bits being set in the PI register. Their bitwise and operation will
still be the value of 'Private->PortBitMap'.

DEPEX from PEI storage stack.

BootInRecoveryMode Ppi dependency should be removed from PEI storage stack
drivers. Besides recovery, more features are using the storage stack.
Platform can apply Ppi dependency in Recovery FV which contains full
storage driver stack. Platform storage stack modules, such as host
controller init PEI module, should also remove this dependency.

Due to the limited resource on the VTd DMA buffer size in the PEI phase,
the driver will limit the maximum transfer block number for 48-bit
addressing.

According to PCDs:
gIntelSiliconPkgTokenSpaceGuid.PcdVTdPeiDmaBufferSize|0x00400000
gIntelSiliconPkgTokenSpaceGuid.PcdVTdPeiDmaBufferSizeS3|0x00200000

The default buffer size allocated for IOMMU mapping is:
* 4M bytes for non-S3 cases;
* 2M bytes for S3

For ATA devices in 48-bit address mode, the maximum block number is
currently set to 0xFFFF. For a device with block size equal to 512 bytes,
the maximum buffer allowed for mapping within AhciPei driver will be close
to 32M bytes. Thus, this commit will limit the 48-bit mode maximum block
number to 0x800, which means 1M-byte maximum buffer for mapping when the
block size of a device is 512 bytes. By doing so, potential failure on
calls to the IOMMU 'Map' service can be avoided.

Ard Biesheuvel 615a72400b MdeModulePkg/PciBusDxe: dispatch option ROMs for foreign architectures
Delete the explicit machine type check for option ROM images, and instead,
rely on the LoadImage() boot service to decide whether an option ROM can
be dispatched or not. This permits platforms to ship with emulators to
execute option ROMs that are not native to the processor architecture.

does not handle the input parameter 'DeviceIndex' properly.

According to both of the PI spec and the function description comments:

> DeviceIndex    Specifies the block device to which the function wants
>                to talk. ... This index is a number from one to
>                NumberBlockDevices.

But current codes incorrectly treat the valid range of 'DeviceIndex' as 0
to (NumberBlockDevices - 1).

This commit is to address this issue.

This commit will add the AHCI mode ATA device support in the PEI phase.

More specifically, the newly add AhciPei driver will consume the ATA AHCI
host controller PPI for ATA controllers working under AHCI code within the
system. And then produces the below PPIs for each controller:

EDKII PEI ATA PassThru PPI
Storage Security Command PPI

Also, the driver will consume the S3StorageDeviceInitList LockBox in S3
phase. The purpose is to perform an on-demand (partial) ATA device
enumeration/initialization on each controller to benefit the S3 resume
performance.

The implementation of this driver is currently based on the below
specifications:
Serial ATA Revision 2.6
Serial ATA Advanced Host Controller Interface (AHCI) 1.3.1
AT Attachment with Packet Interface - 6 (ATA/ATAPI-6)

Hao Wu 4104423ac0 MdeModulePkg/NvmExpressPei: Avoid updating the module-level variable
This commit is out of the scope for BZ-1409. The commit will remove the
call of RegisterForShadow() at the entry point of the driver. By doing so,
the driver is now possible to be executed without being re-loaded into
permanent memory.

Thus, this commit will update the NvmExpressPei driver to avoid updating
the content of a global variable.

In eMMC HS400 switch sequence flow eMMC driver attempted
to execute SEND_STATUS just after switching bus timing to high
speed and before downgrading clock frequency to 52MHz. Since link
was at that time in incorrect state SEND_STATUS was failing which
made driver think switch to HS400 failed.
This change makes driver always change clock frequency after
switching bus timing and before executing SEND_STATUS.

Ray Ni 0fa92d5839 MdeModulePkg/PciBus: Fix a bug PPB MEM32 BAR isn't restored sometimes

When a device under PPB contains option ROM but doesn't require 32bit
MMIO, ProgrameUpstreamBridgeForRom() cannot correctly restore the
PPB MEM32 RANGE BAR. It causes the 32bit MMIO conflict which may
cause system hangs in boot.

The root cause is when ProgrameUpstreamBridgeForRom() calls
ProgramPpbApperture() to restore the PPB MEM32 RANGE BAR, the
ProgramPpbApperture() skips to program the BAR when the resource
length is 0.

This patch fixes this issue by not calling ProgramPpbApperture().
Instead, it directly programs the PPB MEM32 RANGE BAR.

Add SDMA, ADMA2 and 26b data length support.

If V4 64 bit address mode is supported in capabilities register,
program controller to enable V4 host mode and use appropriate
SDMA registers supporting 64 bit addresses.

If V4 64 bit address mode is supported in capabilities register,
program controller to enable V4 host mode and use appropriate
ADMA descriptors supporting 64 bit addresses.

If host controller version is above V4.0, enable ADMA2 with 26b data
length support for better performance. HC 2 register is configured to
use 26 bit data lengths and ADMA2 descriptors are configured appropriately.

ROM when there is PCI option ROM.

Ruiyu Ni 0176af142e MdeModulePkg/PciBus: Shadow option ROM after BARs are programmed

Today's implementation reuses the 32bit MMIO resource requested by
all PCI devices MMIO BARs when shadowing the option ROM.
Take a simple example, a system has only one PCI device. It requires
8MB 32bit MMIO and contains a 4MB option ROM. Today's implementation
only requests 8MB (max of 4M and 8M) 32bit MMIO from
PciHostBridgeResourceAllocation protocol. Let's assume the MMIO range
[3GB, 3GB+8MB) is allocated. The 3GB base address is firstly
programmed to the option ROM BAR for option ROM shadow. Then the
option ROM decoding is turned off and 3GB base address is programmed
to the 32bit MMIO BAR.

It doesn't cause issues when the device doesn't request too much
MMIO.
But when the device contains a 64bit MMIO BAR which requests 4GB MMIO
and a 4MB option ROM. Let's assume [3GB, 3GB+8MB) 32bit MMIO range is
allocated for the option ROM. When the option ROM is being shadowed,
64bit MMIO BAR is programmed to value 0, which means [0, 4GB) MMIO is
given to the 64bit BAR.
The range overlaps with the option ROM range which may cause the
device malfunction (e.g.: option ROM cannot be read out) when the
device has two separate decoders: one for MMIO BAR, the other for
option ROM.

The patch requests dedicated MEM32 resource for Option ROMs and
moves the Option ROM shadow logic after all MMIO BARs are programmed.
The MMIO BAR setting to 0 when shadowing Option ROM is also skipped
because the MMIO BAR already contains the correct value.

Marcin Wojtas 7f3b0bad4b MdeModulePkg/SdMmcPciHcDxe: Allow overriding base clock frequency
Some SdMmc host controllers are run by clocks with different
frequency than it is reflected in Capabilities Register 1.
It is allowed by SDHCI specification ver. 4.2 - if BaseClkFreq
field value of the Capability Register 1 is zero, the clock
frequency must be obtained via another method.

Because the bitfield is only 8 bits wide, a maximum value
that could be obtained from hardware is 255MHz.
In case the actual frequency exceeds 255MHz, the 8-bit BaseClkFreq
member of SD_MMC_HC_SLOT_CAP structure occurs to be not sufficient
to be used for setting the clock speed in SdMmcHcClockSupply
function.

This patch adds new UINT32 array ('BaseClkFreq[]') to
SD_MMC_HC_PRIVATE_DATA structure for specifying
the input clock speed for each slot of the host controller.
All routines that are used for clock configuration are
updated accordingly.

This patch also adds new IN OUT BaseClockFreq field
in the Capability callback of the SdMmcOverride,
protocol which allows to update BaseClkFreq value.

Some SD Host Controllers use different values in Host Control 2 Register
to select UHS Mode. This patch adds a new UhsSignaling type routine to
the NotifyPhase of the SdMmcOverride protocol.

UHS signaling configuration is moved to a common, default routine
(SdMmcHcUhsSignaling). After it is executed, the protocol producer
can override the values if needed.

The fix is similar to commit ebb6c7633b.
We found that a similar fix should be applied to the NVMe PEI driver as
well. Hence, this one is for the PEI counterpart driver.

According to the the NVM Express spec Revision 1.1, for some commands
(like Get/Set Feature Command, Figure 89 & 90 of the spec), the Memory
Buffer maybe optional although the command opcode indicates there is a
data transfer between host & controller (Get/Set Feature Command, Figure
38 of the spec).

Hence, this commit refine the checks for the 'TransferLength' and
'TransferBuffer' field of the
EDKII_PEI_NVM_EXPRESS_PASS_THRU_COMMAND_PACKET structure to address this
issue.

When (Len < Offset) is TRUE, indicating the data to visit is beyond
the boundary, the error message is printed but the function doesn't
return NULL.

descriptor is valid before using it.

Ruiyu Ni c96de1dbae MdeModulePkg/UsbBusPei: Fix out-of-bound read access to descriptors
Today's implementation reads the Type/Length field in the USB
descriptors data without checking whether the offset to read is
beyond the data boundary.

The patch fixes this issue by syncing the fix in commit
4c034bf62c

ParsedBytes in UsbBusPei.GetExpectedDescriptor() is different from
Consumed in UsbBusDxe.UsbCreateDesc().
ParsedBytes is the offset of found descriptor while Consumed is
offset of next descriptor of found one.

For the PassThru() service of NVM Express Pass Through Protocol, the
current implementation (function NvmExpressPassThru()) will only use the
IO Completion/Submission queues created internally by this driver during
the controller initialization process. Any other IO queues created will
not be consumed.

So the value is little to accept external IO Completion/Submission queue
creation request. This commit will refine the behavior of function
NvmExpressPassThru(), it will only accept driver internal IO queue
creation commands and will return "EFI_UNSUPPORTED" for external ones.

Hence, this commit will always copy the content of completion queue entry
to the PassThru packet regardless of the execution result of the command.

Hence, this commit refine the checks for the 'TransferLength' and

structure to address this issue.

Ruiyu Ni 4f8b2f9d72 MdeModulePkg/UsbMass: Fix integer overflow when BlockSize is 1
UsbBootReadWriteBlocks() and UsbBootReadWriteBlocks16() use a UINT16
local variable to hold the value of
USB_BOOT_MAX_CARRY_SIZE (=0x10000) / BlockSize.
When BlockSize is 1, the UINT16 local variable is set to 0x10000
but the high-16 bits are truncated resulting the final value be 0.

It causes the while-loop in the two functions accesses 0 block in
each loop, resulting the loop never ends.

The patch fixes the two functions to make sure no integer overflow
happens.

Ard Biesheuvel 1a3bee2082 MdeModulePkg/NonDiscoverablePciDeviceDxe: expose unique B/D/F identifiers
Currently, the implementation of EFI_PCI_IO_PROTOCOL::GetLocation()
in NonDiscoverablePciDeviceDxe returns the same set of dummy values
for each instance of the NON_DISCOVERABLE_DEVICE protocol that it
attaches itself to. However, this turns out to be causing problems
in cases where software (such as the ARM Compliance Test Suite [ACS])
attempts to use these values to uniquely identify controllers, since
the collisions create ambiguity in this regard.

So let's modify GetLocation() to return an arbitrary bus/device tuple
on segment 0xff instead. This is guaranteed not to clash with other
non-discoverable PCI devices, and highly unlikely to clash with real
PCIe devices.

In current code logic, when a key is pressed, it will search
the whole NotifyList to find whether a notification has been
registered with the keystroke. if yes, it will en-queue the
key for notification execution later. And now if different
notification functions have been registered with the same key,
then the key will be en-queued more than once. Then it will
cause the notification executed more than once.

This patch is to enhance the code logic to fix this issue.

The EhcReset(), EhcGetRootHubPortStatus() and EhcDriverBindingStart()
functions need to see whether the host controller (or a specific port on
the host controller) can be accessed, dependent on the controller having
(or the specific port being) an in-use debug port. Because the condition
isn't simple, extract it to a separate function.

Laszlo Ersek c3d5d800d7 MdeModulePkg/EhciDxe: fix host controller reset condition in BindingStart
Commit 09943f5ecc ("MdeModulePkg: Skip to manage usb debug port in EDKII
EHCI driver if it's used by usb debug port driver", 2012-04-28) made the
host controller reset in EhcDriverBindingStart() conditional. The intent
was to avoid the reset when
- one of the USB ports on the host controller was a Debug Port, AND
- the Debug Port was in use.

This translates to the following positive condition: reset the controller
only if:
- no USB port on the host controller is a Debug Port, OR
- the Debug Port is not in use.

Commit 09943f5ecc failed to implement the first subcondition, and thus
the result is too strict now (for resetting the host controller). Relax it
to the correct condition.

This issue was found by Steven Shi on QEMU. QEMU's EHCI device model does
not have a Debug Port. In repeated disconnect / connect cycles, the
controller is never reset in EhcDriverBindingStart(), therefore the
devices on the controller are never re-enumerated after a disconnect.

Star Zeng aa4240edff MdeModulePkg SataControllerDxe: Calculate ChannelCount based on PI value
Current code calculates ChannelCount based on CAP(NP) value.
It only works when the ports implemented number are <= CAP(NP),
for example, platform has CAP(NP) = 5 (means 6 ports) and ports
implemented are 0, 1, 2, 3, 4 and 5.

But we have some platform that has CAP(NP) = 1 (means 2 ports) and
ports implemented are 1 and 2, and has no port 0 implemented, then
current code does not work.

This patch updates the code to calculate ChannelCount based on PI value.

sizeof(EFI_USB_HUB_DESCRIPTOR).

The patch updates the code to use a big enough buffer to hold the
descriptor data.

sizeof(EFI_USB_HUB_DESCRIPTOR).

The patch updates the code to use a big enough buffer to hold the
descriptor data.
The definition EFI_USB_SUPER_SPEED_HUB_DESCRIPTOR is wrong (HubDelay
field should be UINT16 type) and no code is using it, the patch
removes it.

Booting from USB may fail while the macro USB_BOOT_IO_BLOCKS
set to 128 because the block size of some USB devices are exceeded
512, like some virtual CD-ROM from BMC, the block size is 2048.

* ShellPkg/[hex]edit: use SimpleTextInEx to read console
changes shell edit and hexedit to read input through SimpleTextInEx.

It exposes a issue in UsbKeyboard driver:
Per UEFI Spec,
When interpreting the data from this function (ReadKeyStrokeEx), it
should be noted that if a class of printable characters that are
normally adjusted by shift modifiers (e.g. Shift Key + "f" key) would
be presented solely as a KeyData.Key.UnicodeChar without the
associated shift state. So in the previous example of a Shift Key +
"f" key being pressed, the only pertinent data returned would be
KeyData.Key.UnicodeChar with the value of "F".

UsbKeyboard driver does convert Shift Key + "f" to "F" without the
shift state. But it doesn't do the conversion for all printable
characters, e.g.: Shift Key + "1" --> "!".

The root cause is today's logic to check whether a character is
printable or not is as below:
  if ((KeyDescriptor->AffectedAttribute & EFI_AFFECTED_BY_CAPS_LOCK)
      != 0) {

So it only converts Shift + "a"-"z", but doesn't for Shift + "0"-"9",
and Shift + "["...

The patch updates the check logic as below to fix the issue:

      (KeyDescriptor->ShiftedUnicode != CHAR_NULL) &&
      (KeyDescriptor->Unicode != KeyDescriptor->ShiftedUnicode)) {

The above check is TRUE when the character is printable and
it's *really* affected by Shift key.

Heyi Guo 74d0a339b8 MdeModulePkg/PciHostBridgeDxe: Add support for address translation
PCI address translation is necessary for some non-x86 platforms. On
such platforms, address value (denoted as "device address" or "address
in PCI view") set to PCI BAR registers in configuration space might be
different from the address which is used by CPU to access the
registers in memory BAR or IO BAR spaces (denoted as "host address" or
"address in CPU view"). The difference between the two addresses is
called "Address Translation Offset" or simply "translation", and can
be represented by "Address Translation Offset" in ACPI QWORD Address
Space Descriptor (Offset 0x1E). However UEFI and ACPI differs on the
definitions of QWORD Address Space Descriptor, and we will follow UEFI
definition on UEFI protocols, such as PCI root bridge IO protocol and
PCI IO protocol. In UEFI 2.7, "Address Translation Offset" is "Offset
to apply to the Starting address to convert it to a PCI address". This
means:

1. Translation = device address - host address.

2. PciRootBridgeIo->Configuration should return CPU view address, as
well as PciIo->GetBarAttributes.

Summary of addresses used in protocol interfaces and internal
implementations:

1. *Only* the following protocol interfaces assume Address is Device
   Address:
(1). PciHostBridgeResourceAllocation.GetProposedResources()
     Otherwise PCI bus driver cannot set correct address into PCI
     BARs.
(2). PciRootBridgeIo.Mem.Read() and PciRootBridgeIo.Mem.Write()
(3). PciRootBridgeIo.CopyMem()
UEFI and PI spec have clear statements for all other protocol
interfaces about the address type.

2. Library interfaces and internal implementation:
(1). Base and Limit in PCI_ROOT_BRIDGE_APERTURE are device address.
     It is easy to check whether the address is below 4G or above 4G.
(2). Addresses in PCI_ROOT_BRIDGE_INSTANCE.ResAllocNode are host
     address, for they are allocated from GCD.
(3). Address passed to PciHostBridgeResourceConflict is host address,
     for it comes from PCI_ROOT_BRIDGE_INSTANCE.ResAllocNode.

RESTRICTION: to simplify the situation, we require the alignment of
Translation must be larger than any BAR alignment in the same root
bridge, so that resource allocation alignment can be applied to both

Ruiyu Ni 06e2409667 MdeModulePkg/UsbMass: Retry CMD for MediaChanged sense key
When a USB device reports failure for a CMD and REQUEST SENSE returns
Media Changed key, UsbBootExecCmdWithRetry() stops to retry CMD and
returns EFI_MEDIA_CHANGED to caller.

For this case, the CMD should be retried until success, getting
NoMedia sense key or timeout.
The patch updates UsbBootExecCmdWithRetry() to follow the above
rule so EFI_MEDIA_CHANGED is no longer returned.

UsbBootDetectMedia() is updated accordingly.

Because UsbBootGetParams() is called for new plugged USB storage,
and some USB storage devices may report Media Changed key,
UsbBootGetParams() is updated to treat it as a Success.
This change could fix the issue that some USB storage devices
cannot be detected.

* MdeModulePkg/UsbStorage: Fix "map -r" cannot detect media change

The above commit fixed the following issue:
When system boots to Shell without CDROM inside USB CDROM drive,
and then user inserts the CDROM with Eltorito file system,
"map -r" cannot show the new ELtorito file system.
The commit caused EFI_MEDIA_CHANGED status returned from
UsbBootDetectMedia().

But that fix exposes another issue:
When issuing ReadCapacity command to certain USB key
(Kingston DataTraveler G3 8GB) after it's hot-plugged, USB device
returns STALL error and RequestSense command returns media changed
sense data. (Most of the USB keys return SUCCESS for ReadCapacity
command after hot-plug.)

In below calling stack:
UsbBootIsUnitReady()
 UsbBootExecCmdWithRetry()
  UsbBootExecCmd()
    UsbBootRequestSense()
When USB CDROM is hot-plugged, UsbBootRequestSense() retrieves sense
key (6 = UnitAttention), additional sense code (29h = Power ON).
But it wrongly maps such sense data to Device Error status.
It causes UsbBootExecCmd() executed again.
In the second time call to UsbBootExecCmd(), UsbBootRequestSense()
retrieves sense key (6 = UnitAttention), additional sense code
(28h = media changed).

The above analysis explains why hot-plug USB CDROM cannot be
recognized after below commit:
SHA1 a662afb5b0
* MdeModulePkg/UsbStorage: Fix "map -r" cannot detect media change,
which removes the media changed status check in UsbBootDetectMedia().

The proper fix to this problem is to map the ASC (additional sense
code 29h) properly to success status so that no second call to

Within function NvmExpressPassThru():

The data buffer for the below 2 Admin command:
Create I/O Completion Queue command (Opcode 01h)

are not mapped to the PCI controller specific addresses.

But the current code logic also prevents the below NVM command:
Write (Opcode 01h)

from mapping its data buffer.

Hence, this commit refine the logic to resolve this issue.

The change also benefits certain implementation that depends on
the PciIo handle in PciEnumerationComplete callback.

Ruiyu Ni a662afb5b0 MdeModulePkg/UsbStorage: Fix "map -r" cannot detect media change
When system boots to Shell without CDROM in the USB CDROM drive,
and then user inserts one CDROM with Eltorito file system,
"map -r" cannot show the the new Eltorito file system.

The root cause is when "map" command probes the media change
by calling dummy ReadBlockIo(), UsbMassStorage ReadBlockIo()
contains a bug that ignores the media change status:
UsbBootDetectMedia() ignores the EFI_MEDIA_CHANGED status
returned from UsbBootIsUnitReady(), in consequence, the
BlockIo protocol re-installation logic doesn't run.

By checking the code change history, below commit
SHA-1: 19bc852765
* remove unnecessary retry logic from usb mass storage driver.
adds the code to skip EFI_MEDIA_CHANGED status.

Talking with the original committer Tian Feng, he said the
change was just a code cleanup, not for a special bug fix.
So I revert part of the patch, only keep the change that
removes unnecessary retry logic. It can fix this bug.

It didn't cause big issues when VT-d was disabled.
But in VT-d enabled platform, lack of EhcFreeUrb call caused

Commit f6b139b added return status handling to PciIo->Mem.Write.
However, the second status handling will override EFI_DEVICE_ERROR
returned in this branch:

Since PciIo->Mem.Write will probably return SUCCESS, it causes
NvmExpressPassThru to return SUCCESS even when DEVICE_ERROR occurs.
Callers of NvmExpressPassThru will then continue executing which may
cause further unexpected results, e.g. DiscoverAllNamespaces couldn't
break out the loop.

So we save previous status before calling PciIo->Mem.Write and restore
the previous one if it already contains error.

If a USB Mass Storage device does not support the Get
Max LUN command, then the USB I/O Protocol ControlTransfer()
service may return an error.  If an error is returned for
this command, then assume that the device does not support
multiple LUNs and return a maximum LUN value of 0.

The USB Mass Storage Class Specification states that a
maximum LUN value larger than 0x0F is invalid.  Add
a check to make sure this maximum LUN value is in this
valid range, and if it is not, then assume that the
device does not support multiple LUNs and return a
maximum LUN value of 0.

This change improves compatibility with USB FLASH drives
that do not support the Get Max LUN command or return
an invalid maximum LUN value.

The USB I/O Protocol function ControlTransfer() has a DataLength
parameter that specifies the size of the Data buffer.  The
UsbBusDxe module implements the USB I/O Protocol using the
services of the USB2 Host Controller Protocol.  The DataLength
parameter in the USB2 Host Controller Protocol ControlTransfer()
service is an IN OUT parameter so the number of bytes actually
transferred is returned.  Since the USB I/O Protocol
ControlTransfer() service can not return the number of bytes
actually transferred, the only option if the number of bytes
actually transferred is less than the number of bytes requested
is to return EFI_DEVICE_ERROR.

The change fixes an issue with a USB mass storage device that
responds with 0 bytes to the Get MAX LUN command.

A USB flash drive is returning Request Sense Key of
USB_BOOT_SENSE_UNIT_ATTENTION and an Additional Sense Key of
USB_BOOT_ASC_NO_MEDIA for a few seconds before returning an
Additional Sense Key of USB_BOOT_ASC_MEDIA_CHANGE.

The current logic treats this initial Request Sense info as an
error and reties the command 5 times before failing completely.

With this change the USB Flash Drive works correctly.

Update logic to not return an error from UsbBootRequestSense()
if a Request Sense command responds with no sense information.
It is legal for a USB mass storage device to respond to a
Request Sense command with a SenseKey of
USB_BOOT_SENSE_NO_SENSE and an Additional Sense
Code of USB_BOOT_ASC_NO_ADDITIONAL_SENSE_INFORMATION.

This is described in Section 3.3 of the Universal Serial
Bus Mass Storage Specification For Bootability:

The previous logic returned an error of EFI_NO_RESPONSE
and this caused USB mass storage devices such as a USB
floppy drive to not be detected.

Heyi Guo 710d9e69fa MdeModulePkg/NonDiscoverable: fix memory override bug
For PciIoPciRead interface, memory prior to Buffer would be written
with zeros if Offset was larger than sizeof (Dev->ConfigSpace), which
would cause serious system exception.

So we add a pre-check branch to avoid memory override.

The patch ensures all DMA transactions are blocked after
ExitBootService.
If a platform enables IOMMU before and needs disable IOMMU after

Ard Biesheuvel 6743455e34 MdeModulePkg/SdMmcPciHcDxe: call SdMmcFreeTrb() to complete sync operation
Currently, we complete a synchronous operation without unmapping the
DMA mappings, and free the pages using FreePages () rather than calling
EFI_PCI_IO_PROTOCOL::FreeBuffer. This is simply incorrect, but it also
breaks non-coherent DMA as well as DMA protection and/or memory encryption
so let's do it correctly and call SdMmcFreeTrb() instead.

Ruiyu Ni b5cbef4eff MdeModulePkg/PciBus: Fix bug that doesn't produce BusOverride
It's a regression of below commit:
SHA-1: 8be37a5cee
* MdeModulePkg/SecurityStubDxe: Defer 3rd party image before EndOfDxe

When PciBus driver fails to load the Option ROM, it doesn't produce
BusOverride protocol. It was a correct behavior before the above
commit. But due to the above commit, BusOverride protocol never is
produced by PciBus driver.

The patch fixes this issue using the following solution:
1. PciBus records the image device path when LoadImage fails.
2. Override.GetDriver() tries to look for the image handle using
   the stored image device path.

Laszlo Ersek 76fd5a660d MdeModulePkg/AtaAtapiPassThru: disable only BM-DMA at ExitBootServices()
Clearing I/O port decoding in the PCI command register at
ExitBootServices() breaks IDE boot in Windows, on QEMU's "pc" (i440fx)
machine type. (AHCI boot on "q35" is unaffected.) Windows seems repeatedly
stuck, apparently waiting for a timeout of sorts.

This is arguably a Windows bug; a native OS driver should not expect the
firmware to leave the PCI command register in any particular state.

Strictly speaking, we only need to disable BM-DMA at ExitBootServices(),
in order to abort pending transfers to/from RAM, which is soon to be owned
by the OS. BM-DMA is also the only bit that's explicitly named by the UEFI
Driver Writers' Guide, for clearing at ExitBootServices().

I've verified that clearing only BM-DMA fixes the issue (boot time) on
i440fx, and does not regress q35/AHCI.

Ruiyu Ni 6e32874427 MdeModulePkg/PciBus: Fix bug that PCI BUS claims too much resource
The bug was caused by 728d74973c
"MdeModulePkg/PciBus: Count multiple hotplug resource paddings".

The patch firstly updated the Bridge->Alignment to the maximum
alignment of all devices under the bridge, then aligned the
Bridge->Length to Bridge->Alignment.
It caused too much resources were claimed.

The new patch firstly aligns Bridge->Length to Bridge->Alignment,
then updates the Bridge->Alignment to the maximum alignment of all
devices under the bridge.
Because the step to update the Bridge->Alignment is to make sure
the resource allocated to the bus under the Bridge meets all
devices alignment. But the Bridge->Length doesn't have to align
to the maximum alignment.

The current implementation assumes there is only one hotplug resource
padding for each resource type. It's not true considering
DegradeResource(): MEM64 resource could be degraded to MEM32
resource.

The patch treat the resource paddings using the same logic as
treating typical/actual resources and the total resource of a bridge
is set to the MAX of typical/actual resources and resource paddings.

Within function AhciModeInitialization(), left shift operations of 'BIT0'
in the following statements:
"if ((PortImplementBitMap & (BIT0 << Port)) != 0) {"

will incur possible out of range left shift when Port is 31, since
"1 << 31" is possible to exceed the range of type 'int' (signed).

According to the C11 spec, Section 6.5.7:
> 4 The result of E1 << E2 is E1 left-shifted E2 bit positions; vacated
>   bits are filled with zeros. If E1 has an unsigned type, the value
>   of the result is E1 * 2^E2 , reduced modulo one more than the
>   maximum value representable in the result type. If E1 has a signed
>   type and nonnegative value, and E1 * 2^E2 is representable in the
>   result type, then that is the resulting value; otherwise, the
>   behavior is undefined.

This commit explicitly cast 'BIT0' with UINT32 to resolve this issue.

The AtaAtapiPassThru driver maps three system memory regions for Bus
Master Common Buffer operation on the following call path, if the
controller has PCI_CLASS_MASS_STORAGE_SATADPA class code:

  AtaAtapiPassThruStart()
    EnumerateAttachedDevice()
      AhciModeInitialization()
        AhciCreateTransferDescriptor()

The device is disabled (including Bus Master DMA) when the controller is
unbound, in AtaAtapiPassThruStop(). Then the regions are unmapped.

The former step should also be done when we exit the boot services, and
the OS gains ownership of system memory.
